Flyer for LPHS 2025 Physical Night. Event is Thursday, May 15, from 4:00 to 7:00 PM at the LPHS Gym. Cost is free. Physical packets must be picked up in advance from the front office and must have "revised 2/25" in the top right corner to be valid. Important note: All paperwork must be dated May 1, 2025, or later for it to count for the next year. A notary will be available at the event. Physical packets must have all signature lines completed by both athlete and parent by May 16. Parents may sign and notarize forms before the event. Athletes may attend alone if all parent signatures on the FHSAA Physical are completed. Only the physical exam can be done without a parent. Other forms requiring a notary need the parent present. If a parent is present, all forms can be completed on-site. Athletes cannot complete the physical without either pre-completed signatures or a parent being present. To complete the process, parents must upload all documents to Athletic Clearance for the 2025–26 school year, starting May 19. All documents are kept by the athlete—nothing is turned in.
